Workers' Compensation
An injury must be emailed to HR within three business days after the incident has occurred. An Injury Report form needs to be provided to HR following an injury regardless of whether or not treatment was sought.
Employees must obtain an Authority for Treatment form, signed by a supervisor, before seeking medical treatment. It is mandatory that all injured employees needing treatment go to the District's Workers' Compensation Medical Provider, either Michigan Urgent Care or a Concentra location of your choice.
Copies of the Injury Report and Authorization for Treatment forms can be found in the administration office of your worksite.
